The ESIC noticed some instances, that the family particulars of IP/IW vary from the particulars that are available in the IP portal. This has lead to undue benefit to the women. In order to eliminate these issues, the ESIC has introduced a Self-declaration form for the insured woman. Now, the insured woman will have to declare the number of her surviving children using this form, as on the date of presenting the benefit claim. If there are any inconsistencies between the declaration and the data present in the portal, then 12 weeks of maternity benefit would be paid immediately from the ESIC branch. The remaining payment, if any, would be paid within 14 days post verification. The insured woman would have to submit the self declaration along with Form 19 for the settlement of Maternity Benefit Claim.
Any False statement or representation for the purpose of obtaining benefit will be punishable with imprisonment for a term which may extend to six months or with a fine of Rs 2000/- or with both.
